Admission Criteria
Rochester Supportive Services, LLC
Individuals seeking services from Rochester Supportive Services, LLC must meet all of the following criteria to be eligible for admission:
1. Waiver Eligibility
The individual must be enrolled in either the Family and Individual Supports (FIS) or Community Living (CL) waiver programs.
Supported Living Residential Services must be covered under one of these waivers.
2. Qualifying Diagnosis
The individual must have a documented developmental disability as defined by the Code of Virginia.
Qualifying diagnoses include, but are not limited to:
Autism Spectrum Disorder
Cerebral Palsy
Asperger’s Syndrome
Pervasive Developmental Disorder
Traumatic Brain Injury
Muscular Dystrophy
Spina Bifida
Spinal Cord Injury
Shaken Baby Syndrome
Spinocerebellar Degeneration
Individuals without one of the listed diagnoses may still be eligible if they have documented significant limitations in two or more adaptive skill areas.
3. Intellectual Functioning
The individual must have an IQ score below 70, based on testing conducted by a licensed psychologist.
Assessments may be obtained through the school system, insurance, or private providers and should be recent (no more than a few years old).
If the individual is over the age of 22 at the time of diagnosis or reassessment, documentation must verify that the disability existed prior to age 22. Acceptable documentation includes:
An Individualized Education Program (IEP)
A medical or psychological statement from a qualified professional
4. Functional Limitations
The individual must demonstrate substantial limitations in at least two of the following functional areas:
Health and Safety
Communication
Task Learning
Personal Self-Care
Mobility
Behavior Management
Community Living Skills
5. SIS Assessment
A Supports Intensity Scale (SIS) assessment must be completed or scheduled to determine the individual's Tier and Level of Support.
If an SIS assessment has not yet been completed but is scheduled, the individual may still be considered for admission.
Final decisions regarding admission will be made by the CEO, Executive Program Director, or an authorized designee, based on whether the agency can provide appropriate, high-quality, person-centered care.
